Those treatments are now standard for women with breast cancer in the lymph nodes, based on the realization that once the disease reaches the nodes, it has the potential to spread to vital organs and cannot be eliminated by surgery alone.
Experts say that the new findings, combined with similar ones from earlier studies, should change medical practice for many patients. Some centers have already acted on the new information. Memorial Sloan-Kettering Cancer Center in Manhattan changed its practice in September, because doctors knew the study results before they were published. “This is such a radical change in thought that it’s been hard for many people to get their heads around it,” said Dr. Monica Morrow, chief of the breast service at Sloan-Kettering and an author of the study, which is being published Wednesday in The Journal of the American Medical Association. Doctors and patients alike find it easy to accept more cancer treatment on the basis of a study, Dr. Morrow said, but get scared when the data favor less treatment.
The new findings are part of a trend to move away from radical surgery for breast cancer. Rates of mastectomy, removal of the whole breast, began declining in the 1980s after studies found that for many patients, survival rates after lumpectomy and radiation were just as good as those after mastectomy.
The trend reflects an evolving understanding of breast cancer. In decades past, there was a belief that surgery could “get it all” — eradicate the cancer before it could spread to organs and bones. But research has found that breast cancer can begin to spread early, even when UGG Nightfall tumors are small, leaving microscopic traces of the disease after surgery.
The modern approach is to cut out obvious tumors — because lumps big enough to detect may be too dense for drugs and radiation to destroy — and to use radiation and chemotherapy to wipe out microscopic disease in other places.
But doctors have continued to think that even microscopic disease in the lymph nodes should be cut out to improve the odds of survival. And until recently, they counted cancerous lymph nodes to gauge the severity of the disease and choose chemotherapy. But now the number is not so often used to determine drug treatment, doctors say. Chemotherapy is recommended, and the drugs are the same, no matter how many nodes are involved.
The new results do not apply to all patients, only to women whose disease and treatment meet the criteria in the study.
The tumors were early, at clinical stage T1 or T2, meaning less than two inches across. Biopsies of one or two armpit nodes had found cancer, but the nodes were not enlarged enough to be felt during an exam, and the cancer had not spread anywhere else. The women had lumpectomies, and most also had radiation to the entire breast, and chemotherapy or hormone-blocking drugs, or both.
The study, at 115 medical centers, included 891 patients. Their median age was in the mid-50s, and they were followed for a median of 6.3 years.
After the initial node biopsy, the women were assigned at random to have 10 or more additional nodes removed, or to leave the nodes alone. In 27 percent of the women who had additional nodes removed, those nodes were cancerous. The new findings are part of a trend to move away from radical surgery for breast cancer. Rates of mastectomy, removal of the whole breast, began declining in the 1980s after studies found that for many patients, survival rates after lumpectomy and radiation were just as good as those after mastectomy.
The trend reflects an evolving understanding of breast cancer. In decades past, there was a belief that surgery could “get it all” — eradicate the cancer before it could spread to organs and bones. But research has found that breast cancer can begin to spread early, even when UGG Nightfall tumors are small, leaving microscopic traces of the disease after surgery.

WASHINGTON — The Obama administration on Wednesday pledged to “immediately intensify” talks with Colombia and Panama on two free trade agreements with the aim of submitting the accords to Congress by the end of the year.
The United States trade representative, Ron Kirk, who made the commitment at a House hearing, also said the administration would submit a revised Air Jordan free trade agreement with South Korea to Congress “in the next few weeks.”
That accord, which was first signed in 2007, hit a roadblock with Democrats that was cleared up with South Korea last December.
At the hearing, members of the new Republican majority in the House accused the White House of dragging its feet on the Colombia agreement, signed in 2006, and Panama agreement, signed in 2007, but did not move forward because of opposition Air Jordan 9 among Democrats in Congress.
Democrats reiterated objections to completing the two agreements: a legacy of violence against union organizers in Colombia, and weak laws in Panama on labor protections and tax enforcement. Supporters of the agreement say the problems have mostly been rectified, but Mr. Kirk said the concerns were significant.
“There remain serious issues to be resolved before these agreements can be submitted for Congressional consideration,” he said. “And some of these issues go to our core U.S. Christian Louboutin values and interest, such as the protection of labor rights. States Diverge on How to Deal With Health Care Ruling

Aides to Mr. Walker, a Republican, did not respond to a request for comment.
Jon Hanian, a spokesman for Idaho’s Republican governor, C. L. Otter, said that state now might cancel a meeting scheduled for March to study the establishment of a health insurance exchange, which the law requires in 2014.
“I think it’s fair to say it’s an open question whether they’ll do that,” Mr. Hanian said. “We’ve had two rulings now that have struck this thing down.”
But Democratic governors tended to echo Gov. Peter Shumlin of Vermont, who said his state was “moving ahead at full speed” to carry out the law. And in many states, even Republican officials who are party to the lawsuit chose to tread cautiously.
“We’ll be required to move forward until such time relief is granted or an appellate decision is finalized,” said Gov. Nathan Deal of Georgia. “It’s a victory, but not a complete victory.”
Judge Roger Vinson of Federal District Court in Pensacola, Fla., ruled Monday that the health care act’s requirement that Americans obtain insurance exceeded Congress’ authority to regulate interstate commerce. Unlike a Virginia judge who invalidated that provision Air Jordan 9 in December, Judge Vinson struck down the entire health care act. Two other federal judges have upheld the law.
Judge Vinson noted that Congress had failed to include language in the bill to legally sever portions found unconstitutional from the rest. That, the judge wrote, left him to determine whether other components could function independently of the insurance mandate and whether Congress would have approved the act without that provision.
Comparing the law to “a finely crafted watch,” Judge Vinson wrote that there were “too many moving parts” for him to “dissect out the proper from the improper, and the able-to-stand-alone from the unable-to-stand-alone.”
He issued a declaratory judgment against the entire law, and suggested that the Obama Giuseppe zanotti boots administration view it as the “functional equivalent” of an injunction to suspend its implementation.
The judge acknowledged in a footnote that his reasoning on the question borrowed heavily from a friend of the court brief submitted by the Family Research Council, a conservative Christian advocacy group.

Among other tasks, federal health officials are working to define “essential benefits” to be provided by all insurers, writing regulations for a new long-term care insurance program and weighing dozens of requests from states, employers and labor unions for exemptions from various requirements.
“There has not been any effect on the work being done by this department,” said Richard Sorian, a spokesman for the Department of Health and Human Services.
In Congress, the Senate Republican leader, Mitch McConnell of Kentucky, put forward the repeal measure as an amendment to an aviation industry bill that is now on the Senate floor.

“No,” comes the reply — not from the waitress but rather the occupant of the next stool, a man in a frayed trucker cap. “I’m sweetie.”
“Sunshine,” says the guy one stool over.
“Precious,” says the woman next to him.
“Cupcake,” says a woman in a nearby booth.
A burly man on the last stool hesitates, and then says, “Sugar britches.”
When the waitress serves the chicken wraps with a “There you go, hon,” the man who ordered ugg boots says, “Pass the salt, sweetie,” to the trucker, who responds, “Sure, hon.”
The commercial closes with the new slogan for Denny’s: “America’s diner is always open.”
In four spots that premiered recently, Denny’s, which has for much of its 58-year history referred to its establishments as “family restaurants,” eschews that term for “diners.”
Frances Allen, chief marketing officer of Denny’s, said consumers in focus groups were not calling Denny’s a family restaurant.
“People think of the brand as a diner, with great comfort food at a great price, and they feel that incredible warmth and incredible connection to the servers,” Ms. Allen said. “There’s a UGG Nightfall to a diner that is very authentic, very warm, very accepting.”
Denny’s will spend an estimated $50 million to $60 million in 2011 for the campaign, the first for the brand by Gotham, an agency that is part of the Interpublic Group of Companies. Denny’s spent $61.1 million on advertising in the first nine months of 2010, compared with $118.8 million spent by Applebee’s, one of its competitors, according to the most recent data available from Kantar Media, a unit of WPP.

To play up the diner theme, the “dinersodes,” as the agency refers to the spots, feature counters more prominently than booths, and depict friendly strangers interacting.
In another spot, which like others highlights value-menu items costing from $2 to $8, a young man explains to his friend on the next stool: “So we have $10. We can get one thing for $8 and one UGG Nightfall for $2, or one thing for six and one for four.”
“Or you could get two things for $4 and one for two,” interjects a man on a nearby stool, gesturing to his plates.
“Or five things for two,” says a man at another stool, with five plates.
“Or one for six and two for two,” adds a mother with two small children from a booth.
The camera returns to the young man who draws his opening hands away from his ears, as if his head were exploding.
The new slogan, “America’s diner is always open,” is meant to connote not merely the kitchen never closing, but also to “open being our attitude and philosophy,” said Peter McGuinness, chief executive of Gotham. Actors in the spots represent a MBT Fanaka swath of ethnicities, ages, and socioeconomic classes.
“We’re open to whomever you are, and we greet customers with open arms and have an open mind,” said Mr. McGuinness.
Harold Butler first opened Danny’s Donuts in Lakewood, Calif., in 1953, changing the name six years later to Denny’s because another chain was called Donut Dan’s.
Its signature breakfast combination plate, the Grand Slam, was first introduced in an Atlanta Denny’s in 1977 as an homage to Hank Aaron, who had broken Babe Ruth’s home run record while playing for the Atlanta Braves three years earlier.
Today there are more than 1,600 locations across the United States and internationally, with 15 percent owned by the company and the remainder by franchisees.
In 2009 and 2010, Denny’s announced in Super Bowl ads that they would serve free Grand Slam breakfasts on a given day the following week. Both years, about two million customers crowded into the restaurant for the free MBT sport 2 shoes, but the chain will neither advertise during the Super Bowl this year nor offer the giveaway.
This is not the first time Denny’s has evoked diners. In the late 1990s, it adopted the slogan “America’s original breakfast diner,” and encouraged franchisees to remodel restaurants with stainless steel exteriors, chrome interiors, neon lighting and checkerboard tile floors.
When many balked at the cost of such renovations, the company suggested a less expensive option with flourishes like jukeboxes and outfitting servers in bowling shirts. By the end of 2000, 342 Denny’s had adopted this approach, while another 64 made the switch to the steel exteriors, according to Denny’s 2000 annual report.

That report projected “systemwide remodeling of Denny’s” to the diner concept “over the next few years,” but the chain abandoned the plan, along with the slogan, a few years later. Today, 55 retro Denny’s diners with the steel exteriors remain, all but one owned by franchisees.
Mr. McGuinness, of Gotham, stressed that for the new campaign there is nothing nostalgic about the ads.
“We’re talking about a diner not in the physical sense per se but in a much larger sense, more as a symbol and metaphor,” he said.
Michael Stern, co-author with his wife, Jane Stern, of “Roadfood,” a guide to local eateries across the country, and co-founder of Roadfood.com, said there was a time when, far from having positive associations, restaurateurs avoided the word “diner” at all costs.
“In the ’20s, ’30s, and ’40s, for any restaurant that strove to serve better food the term ‘diner’ was anathema,” Mr. Stern said. “It meant greasy food and the dark and dirty MBT Nuru side of eating out.”
As for whether the term suited Denny’s, Mr. Stern was dubious.
“For most people who think about food at all, when they think of a diner, they think of a place with a unique personality,” Mr. Stern said. “And obviously by definition that cannot be a chain restaurant because it’s not unique.”
